Abstract

A 57-year-old woman was seen at the hospital because of a perianal mass. An elastic hard mass was present in the left-lower lesion of the coccyx. CT and magnetic resonance imaging scans demonstrated a mutilocular cystic mass in the left posterior surface of the rectum and in the presacral space. With a strong suspician of a tailgut cyst, the mass was dissected with transsacral approach as a presacral cystic tumor. The mass was 6×5×4cm in dimension, had smooth surface, and was filled with pasty substance. Histologically, the cyst wall was composed of fibrous connective tissue, and lined with squamous epithelium inside. No muscle layer nor serosa was detected in the wall, and we diagnosed the cyst as tailgut cyst. Tailgut cysts are rare and only 21 cases have been reported in Japan. With the spread of the term tailgut cyst, it may be more frequent in the presacral cystic tumors.
