The 5-year rough survival rate of hypopharyngeal cancer N2 cases was 32% and prognosis was poor. The clinical stage of N2 was the outcome for which we could not classify prognosis precisely. However the number of positive lymph nodes (pN) was related with survival rate and the factor which reflected prognosis. As the number of pN increased, the rate of cases with extra-capsular invasion, level IV lymph node metastases, tended to increase. There were many distant metastases for patients with 2 lymph node metastases, whereas, there was much cervical recurrence with more than 5 lymph node metastases. For patients with more than 5 lymph node metastases, radical cure is difficult by a traditional treatment. It was good prognosis and indication of radical operation for patients with 0, 1 lymph node metastasis, and with 2 or 3 lymph node metastases to have no extra-capsular invasion, level IV lymph node metastases. For the latter, we thought that prevention of distant metastasis was a future problem.
