A STUDY OF PARTIAL GLOSSECTOMY BY LASER SURGERY FOR SQUAMOUS CELL CARCINOMA OF THE TONGUE (T1,T2N0)

Abstract

A total of 37 patients who had squamous cell carcinoma of the tongue (T1, T2N0) underwent partial glossectomy by laser surgery at the Department of Oral and Maxillofacial Surgery, Kobe University Graduate School of Medicine, between 1981 and 2002. They consisted of 22 of T1 and 15 of T2. Twenty-three of the 37 patients were males and 14 were females. The mean age was 61.8 years, ranging from 39 to 79 years. Laser surgery was able to control hemorrhaging during the operation. It was also able to reduce postoperative pain and edema because of minimal damage to adjacent tissue; this keeps functional disorders to a minimum. Of the 37 patients, local recurrence and regional lymph node metastasis were observed in 5 (13.9%) and 3 (8.1%), respectively. Four of 5 patients in whom local recurrence was observed were T1 cases. Two of 3 patients who had regional lymph node metastasis were also local recurrence cases, and died from regional lymph node metastasis. It is suggested that partial glossectomy by laser surgery is correctly indicated in any superficial tongue cancer leaving no postoperative dysfunction. However, the indication of laser surgery for T1, T2N0 tongue cancer is limited to cases that have invaded the muscle by less than 7mm.
