In our previous reports, we described that invasion into the muscular layer may be the most reliable parameter to predict subsequent lymph node metastases in squamous cell carcinomas of the tongue. But any significant histological differences between patients developing cervical metastases and those without metastases could not be found by comparisons of the degree of muscular invasion, tumor differentiation, vascular invasion and perineural spread. The p53 overexpression have been reported to be found in a high percentage of head and neck squamous cell carcinomas.<br>To determine p53 overexpression may be useful or not as a predicting factor for cervical lymph node metastases in patients with tongue carcinoma invading into the muscular layer, nineteen patients with T1N0 M0 squamous cell carcinoma of the oral tongue treated with a partial glossectomy alone (10 with subsequent neck metastasis versus 9 without) were analyzed. The incidence of subsequent cervical node metastases in patients positive for p53 overexpression and those negative was 73% (8/11) and 25% (2/8), respectively.<br>The higher incidence of nodal metastasis for patients with p53 overexpression was not associated with tumor differentiation. In conclusion, these results indicated that p53 overexpression in the tumor cells of the muscular layer may be an useful parameter to predict cervical nodal metastases in squamous cell carcinoma of the oral tongue.
