抄録

A 69-year-old man referred for abdominal fullness due to an abdominal tumor was found in abdominal CT to have a giant tumor adjacent to the stomach, duodenum, and transverse colon. Angiography showed a hypervascular tumor fed by the right gastroepiploic artery, left gastric artery, and right colic artery. We conducted surgery under a diagnosis of gastrointestinal stromal tumor (GIST) originating in the stomach or omentum. Pancreatoduodenectomy with resection of the transverse colon was done because the tumor had invaded the pancreas and transverse colon. The tumor was 18×16×13 cm and weighed 3, 250 g. Pathological findings showed diffuse proliferation of malignant cells characterized by eccentric nuclei. Immunochemically, IgG and k were positive. Therefore we diagnosed the tumor as plasmacytoma. Postoperative bone marrow puncture was unremarkable.
