Most bowel injuries caused by blunt abdominal trauma are acute, such as a rupture or tear, and delayed large bowel obstruction is rare. We report a case of delayed large bowel obstruction following blunt abdominal trauma. A 20-year-old man with epigastralgia had been struck in the abdomen by a car steering wheel. On the 17th day following injury, he was hospitalized with appetite loss and abdominal pain. In a barium enema, the contrast medium stagnated in the transverse colon and none passed to the oral side. Laparotomy showed a hard mass in the transverse colon apparently causing the blockage and it was excised. Histopathologically, there was a rupture in the proper muscle divided by granulation tissue and fibrous tissue. Thick fibrosis extended outside the serous membrane, which was thick and scarred.<br>Although the number of unnecessary laparotomy cases following trauma has been decreasing with advances in diagnostic imaging, delayed complications such as ours are likely to increase, necessitating greater attention in follow-up.
