Patient 1: A 63-year-old man underwent laparoscopy-assisted transverse colon excision and reoperation on postoperative day 21 for ileus. A lump formed by mesenteric panniculitis around the anastomosis and jejunum was resected. Ileus from mesenteric panniculitis around the anastomosis of the jejunum occurred postoperatively. When 60 mg/day of prednisolone was administered from day 67 after the second operation, ileus improved and the patient was discharged on day 144.<br>Patient 2: An 86-year-old man underwent sigmoidectomy under minimum incision laparotomy. Ileus appeared on postoperative day 15. CT showed a thickening of fat tissue around the anastomosis and CT increased. When prednisolone was prescribed from day 17, ileus was improved and the patient was discharged on day 39.<br>CT is useful in diagnosing mesenteric panniculitis; a thickening of fat tissue that presents different CT from other parts is a feature of mesenteric panniculitis. Steroid is effective in treatment and medical treatment should be chosen. In ileus occurring soon postoperatively, it is important to keep this disease in mind and to avoid unnecessary surgery.
