救命効果のある40歳代の乳癌検診(5) : 40歳代検診にはMMG, USの併用を Breast Cancer Screening by Both Mammography and Ultrasound is Recommended for Women Aged 40-49y in Japan

当施設における検診成績を検討し, 40歳代の乳癌検診のモダリティとして, MMG, USが有効であるか評価した。総受診者6,556名, 実受診者3,397名のうち乳癌発見率0.73%, 初回受診発見率1.09%であった。年代ごとの発見率は, 30歳3例 (0.82%), 40歳代12例 (0.92%), 50歳代16例 (0.64%), 60歳以上17例 (0.73%) であった。40歳代の乳癌12例のうち, モダリティごとの発見率は, MMGのみ41.7%, USのみ17.6%, MMG, USの2法ともに発見41.7%であった。このことにより, 40歳代においてもMMGは有用である。発見された乳癌発見のうち早期癌が79.2%を占め, 40歳代では早期癌が75%を占めた。発見率, 早期癌率ともに高いのは, MMG, USの併用による効果と考えられ, 特に40歳代において最も併用の効果が大きい。したがって, 40歳代の乳癌検診には, MMG, USの併用が望ましい。

Since 1993 we have been performing breast cancer screening by mammography, ultrasonography and palpation in females visiting our health administration center. We screened 6, 556 patients between April 1993 and March 1999, and 48 cases of breast cancer were detected. The detection rate was 0. 73%. Among patients aged 40-49, 12 cases of cancer were detected - a detection rate of 0.92 %. The detection rates in patients aged over 60, aged 50-59 and aged 30-39 were 0.73%, 0.64% and 0.82%, respectively. It appeared that in patients aged 40-49 there were more augmenting effects than in other age groups. The TMN stages of the 12 cancers detected were : Stage 0 : 50% ; stage 1 : 25% ; and stage 2 : 25%. The rate of detection of breast cancer by mammography and that by ultrasound were not equal in women aged 40-49. Mammography alone detected 5 cases of cancer, and ultrasound alone detected 2 cases. The remaining 5 cases were detected successfully by a combination of both methods. Therefore, combination screening is more effective for early detection. We strongly recommend the joint use of mammography and ultrasound for breast cancer screening.
