A 96-year-old woman was admitted to our hospital for the treatment of nausea and vomiting. She was diagnosed as having an ileus by abdominal X-ray, and treatment using a long tube was started 2 days after admission. Five days later, a small intestinal series showed an obstruction of the small intestine near the left obturator foramen. A CT scan performed immediately after the small intestinal series showed an incarcerated intestine between the pectineus and external obturator muscles on both side.<br>An emergency operation was performed using a peritoneal approach. The jejunum was incarcerated to the left obturator foramen, and the ileum was incarcerated to the right obturator foramen. After the reduction of the incarcerations, both of the hernia portas were closed, and the necrotic ileum was removed using a wedge-shape-resection. The patient was discharged 33 days after the operation.<br>Only 16 cases of synchronous bilateral obturator hernias, including the present case, and 7 cases of metachronous bilateral obturator hernias have been reported in Japan. In patients with obturator hernias, pre-operative and intra-operative investigations to rule-out the possibility of bilateral obturator hernias are important.
