Abstract

Hereditary non-polyposis colorectal cancer (HNPCC) is a genetic disease caused by the mutation of DNA mismatch repair genes. The clinical features of this disease are familial occurrence, young age of the patients, right sided colon cancer, multiple colorectal cancers and the association with HNPCC related cancers. The clinical diagnosis is made by the Amsterdam II criteria, in which the presence of HNPCC related cancers such as endometrial cancer, ureteric cancer and small bowel cancer is an important diagnostic feature. Category A in Japanese criteria is mostly similar to the Amsterdam criteria. However, category B of Japanese cri-teria is used mainly for the purpose of screening. The direct diagnosis is made by the mutation analysis of DNA mismatch repair genes. Surgical treatment of HNPCC is usually similar to the colorectal cancer surgery in general. However, subtotal colectomy is a treatment of choice in cases with a definitive diagnosis of HNPCC. HNPCC patients and the family members at risk should be followed up periodically for colorectal cancer and other HNPCC related cancers.
