An asymptomatic 42-year-old man was found having a cystic tumor of 4.0cm in diameter at the uncus portion of pancreas by abdominal ultrasonography and CT scan. Angiography and MRCP showed no recognizable findings. Laparotomy was performed with a probable diagnosis of a solid-pseudopapil-lary tumor of the pancreas or a retroperitoneal tumor. At laparotomy, a tumor connected with the superior mesenteric plexus was present, and it was removed with a suspicion of a neurogenic tumor. Histopathological findings revealed neurilemoma predominant over Antoni type B.<br> Neurilemoma is a tumor derived from Schwann cells of the peripheral nerve. It commonly appears in the neck and extremities, and sometimes in the retroperitoneum. Neurilemoma growing at the superior mesenteric plexus is rare, and its differentiation from a pancreatic tumor is difficult because of its anatomical adjacency. We need to take this disease into consideration when we encounter a patient with a cystic tumor occurred at the uncus portion of the pancreas.
