We report a case of von Recklinghausen's disease complicated by breast cancer and pheochromocytoma.<br> A 63-year-old woman referred to the hospital because of a left breast lump was diagnosed as having breast cancer after close exploration and scheduled for surgery. However, preoperative abdominal ultrasonography visualized a right adrenal tumor, and café au lait spots and multiple soft tumors were observed in the skin of the whole body. The patient was diagnosed as having left breast cancer and right pheochromocytoma with von Recklinghausen's disease by close examinations. Considering possible risks due to hypertension caused by pheochromocytoma in the intra-and post-operative management, we performed a laparoscopic right adrenalectomy for pheochromocytoma, followed by a left partial mastectomy for breast cancer. Histopathologically the breast tumor was invasive ductal carcinoma, t2n1 (2/8, m0, stage IIB, ER(-), and PR(-).<br> Von Recklinghausen's disease is often complicated by malignant tumors, but the complication of epithelial tumors is relatively uncommon. This case is presented here together with a review of the literature.
