SURGICAL RESTORATION WITH A FREE ILEOCOLIC GRAFT AFTER LARYNGOPHARYNGECTOMY

Abstract

Eight patients with hypopharyngeal carcinoma underwent surgical voice restoration with a free ileocolic graft after laryngopharyngectomy from July 2003 to July 2004. The mean average age of these patients was 62.1 years old.<BR>The free ileocolic graft operative procedure usually consists of anastomosis of caecum to pharynx, then anastomosis of ascending colon to esophagus, followed by anastomosis of ileum to trachea and, finally, anastomosis of feeding vessels. A tonometer was used to monitor the free ileocolic graft. Thus far, the order of the anastomosis seemed to have little influence on the graft survival.<BR>Five out of eight patients regained their voice and the remaining three underwent training for voice restoration. Postoperative complications were as follows: one vascular pedicle disruption, three leakages at the anastomosis of the graft to esophagus and four troubles around the stoma of tracheostomy. There were no major complications such as aspiration, dysphagia, and graft necrosis.<BR>The free ileocolic graft improved swallowing and voice production while preventing aspiration, and is useful for improving quality of life in patients with hypopharyngeal carcinoma.
