The patient was a 65-year-old man undergone a total thoracic esophagectomy and retro-sternal reconstruction using the gastric tube for thoracic esophageal cancer in August 1996. Five years and 9 months after the esophagectomy, endoscopy revealed a depressed lesion (0-II) 1.5cm in diameter at the lower part of gastric tube, and endoscopic biopsy showed poorly differentiated adenocarcinoma. A partial resection of the gastric tube was performed because endoscopic mucosal resection was unable to be indicated for the case. Histopathological findings included moderately differentiated adenocarcinoma, m, ly0, v0, pm(-), and dm(-). As of nine months after the operation, the patient is still disease-free. Ninety-seven cases of gastric tube cancer reported in Japan, including our case, are reviewed and discussed. In early cancers out of these 97 cancers, the five-year survival rate was as good as 71.2%. From the standpoint of inproving therapeutic outcomes for esophageal cancer at present, it is important to follow the patients by periodic endoscopic examination after esphagectomy to find the disease in an early stage.
