The effectiveness of breast cancer screening for female metropolitan public servants was studied by comparing the cancer detection rates and recall rates for further examination as screening methods were modified. About 75% of the examinees were the same as those screened in the previous year and screening was performed yearly.<BR>Between 1989 and 1999, breast cancer was detected in 41 of 18, 879 examinees. In 1989 and 1990, mass screening for breast cancer was performed by palpation alone. In 1991 and 1992, examinees who were over 45 years of age underwent digital mammoradiography with medio-lateral oblique projection (DMR 1R) in addition to palpation. From 1993 to 1995, screen/film mammography with medio-lateral oblique projection (SMG 1R) was performed for examinees over 45 years old. From 1996, in addition to palpation, examinees under 49 years of age underwent ultrasonography (US) and those over 50 underwent SMG with cranio-caudal and medio-lateral oblique projection (SMG 2R).<BR>The cancer detection rate with palpation alone was 0.1%, which was increased to 0.25% by the addition of DMR 1R or SMG 1R for examinees over 45 and to 0.23% by the addition of US (<50 years) and SMG 2R (≥50years).The recall rate with palpation was 18. 2%, which was reduced to 12.6% and 10.3% by the addition of DMR 1R and SMG 1R, respectively, and it was reduced to 5.5% when both US (<50years) and SMG (≥50 years) were added to palpation.
