We assessed the benefits and limitations of using an omental pedicle flap (OPF) in the treatment of empyema. Ten cases were evaluated with regard to the patients' background, operative procedure, reason for choosing OPF, outcomes and abdominal complications. We applied OPF to patients who showed the followings: hard to close fistulas, insufficient size of muscle pedicle flap to fill the empyema cavity, positive cultures despite chest drainage or open window thoracostomy and those that needed to avoid thoracoplasty. Treatment was successful initially, however later recurrence occurred in two cases with residual empyema walls. OPF was successful even in cases with residual dead spaces or in one-stage operation. In our series, no serious abdominal complications were observed. In conclusion, OPF is useful for the treatment of empyema because of the high success rate in controlling airleakage, even in positive cultures if the bacteria is sensitive to antibiotics and good outcome is expected despite the remaining dead space. However recurrences were also observed. Therefore, a clear understanding of the limitation of the OPF is essential prior to its use. It should be applied according to the indication criteria and following a suitable treatment protocol.
