A 55-year-old man with a previous history of gastric resection 10 years earlier visited the hospital because of abdominal pain and was admitted with a diagnosis of adhesive ileus. Conservative therapy was unsuccessful, and a laparotomy was performed on the 11th hospital day. It was found that the small intestine 130cm apart from the ileocecal valve was adherent to the former surgical scar on the abdominal wall to be narrowed. Then a partial excision of the small intestine was performed. After the operation the patient was prone to have dehydration and, despite sufficient transfusion for the condition, he developed anuria and went into shock on the 3rd postoperative day. Furthermore, erythema appeared in the right lateral area of abdomen since the postoperative day 2 and purpura with bloody bulla developed on the 3rd postoperative day. Multiple organ failure associated with necrotic fascitis was diagnosed and we tried to save the patient's life. However, he died on the same day. Thereafter Aeromonas hydrophilia was isolated from his stools and bloody bulla.<br> Although Aeromonas hydrophila is a weakly toxic microorganism, sometimes it causes septicemia and necrotic fascitis to lead to fulminant condition. We present this case together with a review of the literature.
