A 63-year-old male patient was referred to our hospital from his private physician with the complaint of a mass in his perianal area. An elastic oval tumor of 5.5×3.5cm in size was present on the left side of anus and ultrasonic, CT, and MRI study confirmed the presence of a mass with fatty and cystic element. The mass was removed with a suspicious diagnosis of perianal liposarcoma. The mass extended from perianal subcutaneous layer on the left side and attached to levator ani muscle. The excision was done with a clear margin and the final pathological diagnosis was aggressive angiomyxoma. Approximately 35 cases of aggressive angiomyxoma including our case were reported so far in Japan. However, most of the cases arose in the vulva of female patients and the one arises in the perianal area of male patients is quite rare. We report our case with the review of literatures in Japan.
