A questionnaire study was performed on renal cell carcinoma of dialysis patients diagnosed among 2 years from March 2002. In addition to the conventional questionnaire items, the discovery stage of metastasis and the modality of kidney replacement therapy were investigated in patients with renal cell carcinoma. Moreover, a review of the results of the past 12 questionnaires conducted every 2 year since 1982 is reported. The questionnaire response rate was 62.7%. There were 534 renal cell carcinomas (420 in males, 112 in females and 2 in unspecified) reported this time. There were 478 hemodialysis patients, 28 peritoneal dialysis patients, five patients treated by renal transplantation for more than half of the duration of end stage renal disease and 14 hemodialysis patients being temporarily treated by peritoneal dialysis and functioning graft. The annual incidence was 191 per 100, 000 dialysis patients. SIR of renal cell carcinoma was 14.3 in males and 17.1 in females. Mean age of dialysis patients with renal cell carcinoma was 53.9±10.9 years old, mean duration of dialysis was 145.7±95.0 months. Renal cell carcinoma was found by symptoms in 5.8%. Metastasis was observed in 13.9% and complication of acquired renal cysts was observed in 81.5%. The mean diameter of cancer was 3.53±2.01cm. Among 64 cases of metastasis, there were 47 cases detected at the diagnosis of renal cell carcinoma and 17 cases found after the diagnosis of carcinoma. According to the questionnaire surveys performed between 1982 and 2004, 2, 873 renal cell carcinomas were collected in total. There were 2, 293 males and 574 females. The male to female ratio was 4:1. Mean age was 55.5±11.5 years old, mean duration of dialysis 126.9±84.9 months. Renal cell carcinomas complicated with acquired renal cysts were found in 81.3%. The diameter of cancer was 3.97±2.67cm, and metastasis was observed in 15.4%. Sequential questionnaire surveys showed an increase in patients with renal cell carcinoma, the diameter of cancer showed a tendency to decrease and the metastasis rate also tended to decrease. Since smaller cancer was found by screening, it may have led to a decline in the metastasis rate. In this questionnaire study, the largest numbers of renal cell carcinomas were summed among the 12 past questionnaire studies. The mean duration of dialysis in patients with renal carcinoma has shown an increase in each succeeding questionnaire. Renal cell carcinoma was established as an important complication of long-term dialysis.
