We report a case of invasive lobular carcinoma (ILC) of the breast presented with a metastatic ovarian tumor. A 53-year-old woman underwent the resection of the total uterus, bilateral appendages of the uterus, and omentum with the diagnosis of a left ovarian tumor in the department of gynecology in our hospital. The histopathological diagnosis was metastatic adenocarcinoma in all the resected specimens. Thus she was referred to us for the examination to detect the primary lesion in January 2005. The palpation, mammography, ultrasonography and fine needle aspiration biopsy revealed a mass with the diameter of 2.0cm in the left breast. In March 2005, partial resection of the breast with axillary dissection was performed. The histopathological diagnosis was ILC of the left breast with distant metastases (pT2N3M1: Stage IV). This is an extremely rare case in which a metastastic ovarian tumor was detected before the primary breast lesion was diagnosed.
