Although most popliteal aneurysms are asymptomatic, thrombosis of the aneurysm causes limb ischemia associated with limb loss. We described herein 2 cases of popliteal aneurysm with thrombosis. One case developed acute limb ischemia and underwent resection of the aneurysm with popliteal-popliteal bypass using a posterior approach, and the other case presented intermittent claudication and the aneurysm was resected with a popliteal-popliteal bypass using a medial approach. In the case of acute or chronic limb ischemia, suspicion of a popliteal aneurysm with thrombosis is important, and a CT or ultrasound examination is considered a useful diagnostic modality. The choice of surgical approach may depend on the location of the aneurysm and the site of the bypass. We believe that the resection of the aneurysm using a posterior approach should be used rather than resection of the aneurysm with proximal and distal ligation through a medial approach, because a once-resected aneurysm has a potential risk of regrowth.
