Abstract

We herein report a case of primary cutaneous nocardiosis occurring in a 79-year-old man. Three years previously, the patient noticed the occurrence of an asymptomatic nodule on his left arm after an injury. He was treated with several oral antibiotics, but no effect on the nodule was observed. The nodule gradually increased in size to 28×14cm, and then several other nodules, approximately lcm2 in size, appeared on his left axilla. He was admitted to our hospital on July 30, 1999. From the cultured biopsy specimen, a white round colony was observed and identified as Nocardia blasilliensis. Since there were no significant findings in any other organs, such as the lungs, a diagnosis of primary cutaneous nocardiosis was made. He was successfully treated by the infusion of IPM/CS at a dose of 1 g/day for 21 days followed by the oral administration of sulfamethoxazole/trimethoprim.
