A 50-year-old man, a heavy drinker, admitted to the hospital because of hematemesis and tarry stool and diagnosed as having bleeding from a gastric ulcer, was returned to hemostasis during gastrointestinal endoscopy. He vomited blood again 24 days after the first episode. Gastrointestinal endoscopy showed a submucosal gastric tumor with an ulcer scar. Surgery was conducted to prevent further bleeding and to make the definitive diagnosis. During surgery, the submucosal gastric tumor was judged to be benign under gastrointestinal endoscopy and proximal gastrectomy was conducted. In separation of the tumor from behind the stomach, dark reddish muddy serous fluid was discharged from the broken tumor capsule. The postoperative course was uneventful, and he was discharged from the hospital 30 days after the surgery. No recurrence has been seen as of 18 months after the surgery. The pseudopancreatic cyst was diagnosed because the gastric serosa remained intact and the tumor was proved to arise from the outside of the stomach. Pseudopancreatic cyst presenting with hematemesis which showed similar findings to those of a gastric submucosal tumor is reported.
