Abstract

In the Nippon Medical School Department of Dermatology, 40 examples of anaphylactoid purpura (AP) were diagnosed over a period of four years (July 1996 to May 2000). We classified these patients according to age groups : under 20 years, 20-59 years (adults), and 60 years and over (seniors). We reviewed the clinical information on these cases, with particular attention to adults and seniors. We noted that in young patients there were few joint symptoms and many cases of recurrent purpura, and that the abdominal pain at the onset of AP was seen not only in young patients, but also in adults and elderly patients. An episode of upper respiratory infection rarely preceded the onset of purpura in adult and older subjects ; however, there was an occurrence of purpura in a patient approximately 1 month following surgery. Adult and senior subjects had more episodes of hematuria and proteinuria than the younger group. However, in many cases among the senior group, kidney dysfunction and various other past illnesses and complications were the origins of purpura. We regard that long-term follow-up or kidney biopsy for renal function analyses is more necessary to detect AP in older patients than in younger patients.
