Thirty-seven patients with primary oral squamous cell carcinoma (SCC) over 85 years of age were clinically evaluated. This group accounted for 5.6 % of the total of 663 patients with primary oral SCC treated in our hospital from 1974 to 2004. These consisted of 16 males and 21 females with a mean age of 88.2 years. As for UICC stage classification, 5 cases (13.5%) were stage I, 14 cases (37.8%) were stage II, 7 cases (18.9%) were stage III, and 11 cases (29.7%) were stage IV. In the elderly over 85 years old, gingiva was the most frequent primary tumor site, and 78.4% of patients had systemic medical complications. Overall 5-year cumulative survival rate in the elderly over 85 years was 53.8 %. According to the treatment manner, the cause-specific 5-year survival rate was 85.4 % in the radical surgery treatment group, the cause-specific 14-month survival rate was 11.4% in the palliative treatment group, and the cause-specific 50-month survival rate was 16.7% in the untreated group. However, the treatment outcome of 21 patients over 85 years old who received radical surgery was equal to that of patients under 85 years old. These results suggest that radical treatments including surgery can improve the long-term survival even in patients over 85 years of age.
