Background: Due to recent progress in information technology and telecommunication, we are able to use fiber optic cable networks in the surgical field. We have been developing telementoring between 2 institutes with a fiber optic cable network, and have been applying it for laparoscopy. We report actual cases and assess the feasibility and the safety of this system. Methods: We developed audiovisual signal information transmission with video cameras, monitors, microphones, computers, and a fiber optic cable network. The distance between two institutes was about 20 kilometers. The operator and assistant were young surgeons and the mentor was an expert in laparoscopy. We conducted laparoscopic splenectomy and cholecystectomy under instructions by the mentor via this system. We recorded surgical images and instructions from the mentor, the time course of operative procedures and incidents, and measured the quality of transmitted signals. Result: All operations were performed successfully. No adverse incidents occurred. The quality of the video image from the operating room to the mentor's room was sufficient for surgery. Conclusion: This study demonstrates that telementoring is feasible and safe. This system will be effective for the medical treatment and education of surgeons in remote regions.
