In reality, gangrenous ischemic colitis is recognized as 10-20% of overall ischemic colitis a with high death ratio of 30-50%. We experienced one case in which we were able to save the life of a patient in shock by an emergency operation. An 83 years old female was hospitalized nearby medical facility on the evening prior to her admission to our hospital for overnight observation after she experienced stomachache and nausea. However, the following morning, the patient went into shock and was transferred to our facility. We recognized tenderness in the whole abdomen as an initial symptom upon arrival. Defence was lightly recognized. Through CT observation, we diagnosed the patient as having panperitonitis caused by perforation of ischemic colitis. We conducted an emergency operation. Intraoperatively, we noticed necrosis from ascending to descending colon and removed them totally. The patient remained in shock after the operation, and her condition began to improve after we conducted PMX therapy. She was released from this facility on the 19th days after the operation. We believe that, due to the high death ratio, enforcing an early diagnosis, an early operation, and a treatment like PMX will allow better recovery from gangrenous ischemic colitis.
