Synovial Sarcoma of the Ethmoidal Sinus

Synovial sarcoma is an aggressive soft-tissue tumor that accounts for up to 10% of soft-tissue sarcomas. The head and neck region is the second most common site of involvement, accounting for up to 10% of cases and the majority arise in the paravertebral connective tissue space. Cases of synovial sarcoma involving the paranasal sinuses are rare. This report documents the second recorded case of synovial sarcoma of the ethmoidal sinus. A 60-year-old male with a nasal tumor detected by an otolaryngologist because of intermittent right-sided epistaxis and nasal obstruction. Anterior rhinoscopy revealed a pinkish mass that involved both nasal cavities. Enhanced computed tomography and magnetic resonance imaging showed a homogeneously enhanced mass that filled the upper part of both nasal cavities, adjacent to the anterior base of the skull with bone erosion. Histopathologically, the tumor was diagnosed as the biphasic synovial sarcoma. It was resected with Weber-Ferguson rhinotomy. The present case had neither recurrence nor metastasis without post-surgical radiotherapy in the 24 months since surgery. Based on the findings of this case, post-surgical radiotherapy may thus not be required in cases of highly glandular biphasic synovial sarcoma.
