A CASE OF TRANSOMENTAL HERNIA DIAGNOSED BY LAPAROSCOPY THROUGH AN INGUINAL SAC DURING SURGERY FOR INCARCERATED INGUINAL HERNIA

Abstract

An 81-year-old man with left hemiplegia and dementia was brought into our hospital by ambulance because of abdominal pain and recurrent vomiting, and was found to have a left incarcerated inguinal hernia. Abdominal and pelvic CT scans showed the incarcerated sigmoid colon in the left inguinal hernia sac and localized intestinal obstruction with radial distribution of mesentery. Emergency surgery was conducted under general anesthesia with a diagnosis of small intestinal obstruction associated with left incarcerated inguinal hernia. Because the surgery revealed that the inguinal hernia lesion was not responsible for the small intestinal obstruction, diagnostic laparoscopy was performed through the left inguinal hernia sac. The laparoscopic findings showed transomental hernia with an ischemic small bowel loop, and thus laparoscopy assisted surgery was performed to reduce the strangulated obstruction completely. As resection of small bowel was unnecessary, mesh plug technique was used for repair of the left inguinal hernia. In this case, laparoscopy assisted surgery through the left inguinal sac played a critical role in diagnosis and repair of transomental hernia with incarcerated inguinal hernia.
