抄録

50歳，女性，2008年5月成人Still病と診断された．同年10月prednisolone（PSL）：0.65mg/kg内服中，発熱，背部の皮下膿瘍を認め，肺，脳実質内に空洞を伴い多発する浸潤影が出現，また血液と皮下膿瘍からNocardia farcinicaが同定され播種性ノカルジア症と診断した．sulfamethoxazol-trimethoprim（ST合剤）による治療を開始したが副作用のため中止，imipenem/cilastatin（IPM/CS），amikacin（AMK）を6週間投与したところ病変はほぼ消失した．その後moxifloxacin（MFLX）の内服に切り替え，病変の再燃はない．<br>
