A 74-year-old woman with the history of left lobectomy for intrahepatic cholangiocarcinoma on 1980 was admitted to our hospital with a diagnosis of bile duct tumor. CT demonstrated the tumor located mainly in the anterior segmental bile duct. The defect of 5 cm length in the bile duct and mucin secretion from a papilla of Vater were observed during ERC. Anterior segmentectomy of liver was performed with the diagnosis of mucin-producing intrahepatic cholangiocarcinoma. Histological examination showed the similar histological finding among the specimens of 1980 and 2004. The present tumor was suspected to be metachronous through 24 years. However, it is possible to be recurred because of the histological similarity of the first and second resected tumors, slowgrowing characteristics of the tumor, and the tumor location which close to the surgical margin of the first operation. This case suggests the need of long-term follow-up after the operation for mucin-producing cholangiocarcinoma.
