Abstract

<b>目的</b> : Fiber cell と子宮頸部扁平上皮病変との関連性を検討することで, その意義を解明する.<br><b>方法</b> : 子宮頸部扁平上皮病変 40 例 (高度異形成, 上皮内癌, 扁平上皮癌I期, II期 : 各 10 例) の子宮頸部細胞診標本を対象とした. Fiber cell 出現数, 出現数と組織学的浸潤の深さとの関連, および fiber cell の形態について検討した.<br><b>成績</b> : Fiber cell 出現数は, 高度異形成, 上皮内癌よりも扁平上皮癌で有意に高値 (p<0.01) となり, さらに扁平上皮癌I期よりもII期で高値 (p<0.01) となった. 出現数と組織学的浸潤の深さは, 統計学的に高い関連性は得られなかった (r=0.36). 細胞形態は, 浸潤群に出現する fiber cell のほうが非浸潤群のものと比較して, 細胞および核が大きく, より細長い傾向であった.<br><b>結論</b> : Fiber cell は, 子宮頸部扁平上皮病変の進行度との間に関連性があることが示唆され, 組織学的浸潤の有無や病変の進行度を推定しうる有用な所見であると考えられた.

<b><i>Objective</i></b> : Reportedly, fiber cells are one of the most important cytological findings for uterine cervical squamous cell carcinoma. The aim of this study was to clarify the significance of fiber cells in uterine cervical squamous lesions (precancerous lesions and carcinoma).<br><b><i>Study Design</i></b> : We analyzed 40 cervical smears including 10 cases consisting of four tissue-types (severe dysplasia, carcinoma <i>in situ</i> [CIS], invasive squamous cell carcinoma [SCC] stageI, and SCC stageII). The numbers of fiber cells were counted and compared among the different tissue-types. The diameters of cells and nuclei of the fiber cells were measured and compared with the degree of invasion.<br><b><i>Results</i></b> : Fiber cells were observed in all the cases. We found that the numbers, size, and length-to-breadth ratio of the fiber cells were correlated with the degree of invasion.<br><b><i>Conclusion</i></b> : Our results showed that the analysis of fiber cells is useful for the diagnosis of uterine cervical squamous neoplasia.
