Abstract

Calciphylaxisは,慢性腎臓病(chronic kidney disease:CKD)に合併する骨・ミネラル代謝異常(mineral and bone disorder:MBD)の一型で,その発症要因は不明とされる.今回,われわれは長期透析を受けている白人男性で,下腿皮膚の有痛性多発性潰瘍により受診し,皮膚生検にて確定診断に至った一例を経験した.Calciphylaxisは難治性のCKD合併症であるが,本症例においてはチオ硫酸ナトリウムを投与したところ,有痛性皮膚潰瘍の著明な改善を観察した.本疾患は,CKD-MBDとしてもまれな病態であるものの,長期血液透析患者の増加とともに注目されてきており,透析医にとってその病像を認識する必要があると思われる.

Calciphylaxis is a complication associated with abnormal mineral and bone disorder of chronic kidney disease (CKD-MBD) and the etiology is largely unknown. We encountered a male Caucasian patient with a history of long-term hemodialysis who developed multiple painful skin ulcers on both thighs. The diagnosis was made based on physical findings and skin biopsy, which showed typical findings of calciphylaxis. Together with conventional treatment for CKD-MBD, the patient was given periodic intravenous infusions of sodium tiosulphate, which successfully improved the skin ulcers. Although the incidence is rare, physicians encountering CKD in dialysis practice must be alert to this complication.
