抄録

当施設におけるbody mass index(BMI)からみた重症急性膵炎(SAP)の予後について検討した.SAP 139例におけるBMI(kg/m<sup>2</sup>)の内訳は,25未満が64.7%,25~30が25.2%,30以上が10.1%であった.BMI 25未満(90例)と25以上(49例)の比較では,初期輸液量,base excess,PaO<sub>2</sub>/FIO<sub>2</sub>,CRP,年齢,腹囲,HbA1c,メタボリック症候群(MetS)陽性率に有意差を認めた.BMI 30未満(125例)と30以上(14例)の比較では,初期輸液量,PaO<sub>2</sub>/FIO<sub>2</sub>,年齢,腹囲,HbA1c,MetS陽性率に有意差を認めた.しかし,各々の合併症併発率,入院期間,致命率に有意差を認めなかった.SAPの肥満例は,非肥満例と比較し,年齢層が低く,呼吸障害併発例が多かったが,肥満とSAPの予後との関連性は認められなかった.<br>

Although obesity has been thought to be a prognostic factor for acute pancreatitis, there is a difference in the contribution among the races. We studied the relationship between the prognosis of severe acute pancreatitis (SAP) and body mass index (BMI).<br> We retrospectively investigated 139 patients with SAP (median age, 52 years old; 106 male and 33 female patients) who were admitted to Showa University Hospital from November 2002 through December 2010. Among 139 patients with SAP, those of BMI less than 25kg/m<sup>2</sup> comprised 64.7%, those of BMI 25-30kg/m<sup>2</sup> did 25.2%, and those of BMI more than 30kg/m<sup>2</sup> did 10.1%. There were significant differences in administered fluid volume in the early phase, PaO<sub>2</sub>/FIO<sub>2</sub>, age, abdominal circumference, HbA1c, and the rate of metabolic syndrome between patients of BMI less than 25 or 30kg/m<sup>2</sup> and those of BMI more than 25 or 30kg/m<sup>2</sup>. Complications, total hospital stay and mortality for SAP did not differ significantly between patients of BMI less than BMI 25 or 30kg/m<sup>2</sup> and those of BMI more than BMI 25 or 30kg/m<sup>2</sup>. Obese patients were younger and had more respiratory disturbance than non-obese patients. However, there was no relationship between obesity and the prognosis of SAP.<br>
