病院が行う在宅医療の可能性 Home care management by hospitals

<b>要 旨</b><br><b>はじめに</b> : 病院が主体となって行う在宅医療について検討した. <br><b>方法</b> : 院内に訪問看護ステーションを有し急性期∼在宅医療を行うE病院を対象に実態調査と訪問看護師へのインタビュー調査を行った. <br><b>結果</b> : 訪問診療の開始平均年齢は80.6歳だった. 導入理由により通院不可能型, ターミナル・ケア型に大別され, 前者は要介護度が高く, 後者は死亡率が高かった. また看護師はE病院の行う在宅医療について多診療科の医師に医療技術的な問題を聞くことができる, いつでも状態に応じ入院可能である, 地域へ責任があるといった感想を抱いていた. <br><b>考察</b> : E病院の訪問診療は高齢者, ターミナル患者が多かった. この状況を看護師は肯定的にとらえている可能性があった. 今後は, 診療所と病院の比較検討を行うとともに病院の行う在宅医療にも支援が必要と考えた.

<b><i>Abstract</i></b><br><b>Objective</b> : We investigated the conditions of home medical care managed by one hospital.<br><b>Method</b> : We examined the status of home care in E hospital. The hospital possesses a home care station, in which interviews were conducted with home care nurses.<br><b>Results</b> : The average patient age at the start of home care was 80.6 years. The subjects were divided into two groups : one of patients unwilling to travel to the hospital and one, a terminal care group. The former had lower ADLs than the latter. The death rate in the terminal care group was high. The nurses in the hospital were aware that they could ask the specialist doctors questions about medical technique, and could take care of patients in the hospital on the basis of their symptoms, and they worked with an attitude of responsibility to their community.<br><b>Conclusion</b> : Many aged and terminal patients presented at E hospital, and the nurses there accepted this situation. We considered that it was important for hospitals to be given public assistance, and necessary for comparative studies to be performed regarding the home care carried out by clinics.
