Abstract

外傷性脳損傷患者の多くは神経行動学的変化を生じ, そのために家族介護者の多くが抑うつや不安などの心理学的苦痛を抱えている。本研究の目的は外傷性脳損傷患者の家族介護者の心理学的苦痛の軽減を図るための介入プログラムの効果を検証することである。16 名の参加者に週 1 回, 1 回4 時間で全 5 回の介入プログラムを実施した。内容は外傷性脳損傷の基礎知識, 高次脳機能障害への対応方法, アサーティブネストレーニングを応用したコミュニケーション技法訓練を中心に構成した。効果判定は GHQ-30, SDS, STAI, RAS を評価尺度とし, 介入前後およびフォローアップ (3 ヵ月後と 6 ヵ月後) の時点で分析した。介入前後およびフォローアップの分散分析では SDS は介入前とフォローアップ 6 ヵ月後に, STAI (状態不安) は介入前と介入後における平均値の比較で統計学的に有意な減少を認めた。

   Primary objective : Because neurobehavioral changes occur in most patients with traumatic brain injury, family caregivers suffer from psychological distress such as depression and anxiety. The purpose of this study is to verify the effect of interventional programs to reduce the psychological distress of family caregivers of persons with traumatic brain injury.<br>   Result : The interventional program was conducted over five meetings which were held for four hours and took place once a week, involving a total of 16 persons. The interventional program mainly consisted of providing basic knowledge of traumatic brain injury, ways of treating cognitive dysfunction and training of communication skills applying assertiveness training. Evaluation criteria were GHQ-30, SDS, STAI and RAS as assessment measures and were analyzed before and after the intervention and at three month and six month follow-ups after the interventional program. A considerable reduction of the mean score was statistically recognized in comparison of SDS at pre-intervention and after the six month follow-up and STAI (state anxiety) at pre-intervention and post-intervention in the analysis of variance of pre-and post-intervention and follow-ups.<br>   Conclusions : The result of this study suggests that the interventional program is effective in reducing psychological distress of family caregivers of person with traumatic brain injury.
