Abstract

1975~2008年までの腺様嚢胞癌(ACC)剖検症例10例と,同時期の扁平上皮癌(SCC)剖検症例とを比較した。<br>ACCの部位は硬口蓋3例,下顎骨2例,上顎歯肉1例,舌1例,舌下腺2例,顎下腺1例で,一次症例6例の病期分類はstage I ;1例,stage II;3例,stage IV;2例であった。<br>ACCの死因は6例が原発巣死,4例が遠隔転移死で,頸部転移死はなかった。また,ACC一次症例の平均生存期間は7年5ヶ月で,SCC 125例の2年2ヶ月に比して長かった。頸部リンパ節転移はACCが30%,SCCが65.6%でありACCは低かった。遠隔臓器転移はACCが肺転移を全例に認めたが,SCCは44%であった。<br>ACCは既知の如く,びまん性かつ浸潤性の進展様式を持つ。転移様式は遠隔臓器転移とくに肺転移が多く,原発巣非制御や肺転移を生じても,SCCに比して担癌状態が長期に亘るという臨床的な特徴があった。

We investigated 10 autopsy cases of adenoid cystic carcinoma (ACC) in the oral and maxillofacial region, compared with those of squamous cell carcinoma (SCC), performed during the period from 1975 to 2008. In these cases, ACC had developed in the hard palate in 3 cases, mandible in 2, maxillary gingiva in 1, tongue in 1, sublingual gland in 2, and submandibular gland in 1. Six patients with primary ACC were classified into stage I in 1 case, II in 3, and IV in 2.<br>The causes of death of patients with ACC included: primary lesions in 6 cases and distant metastases in 4. Deaths due to cervical metastases were not observed. The mean survival time for patients with primary ACC was estimated to be as long as 7 years and 5 months, while that of 125 patients with SCC was 2 years and 2 months. The incidence of cervical LN metastases was 30% in ACC and 65.6% in SCC; the former showed a lower rate. Regarding distant organ metastases, lung metastases were observed in all patients with ACC, and 44% of those with SCC.<br>As is known, ACC shows a diffuse invasive growth pattern. Distant organ metastasis occurs, and lung metastasis is frequently observed. As a clinical characteristic, even when the primary lesion becomes uncontrollable, or lung metastasis develops, patients with ACC show long-term survival in a tumor-bearing state compared with those with SCC.
