Complete clinical information on the patient and aspirated lesion, proper aspiration technique, and an adequate sample of well-fixed, correctly stained cellular material for microscopic evaluation are crucial to maintaining a high level of accuracy in fine-needle aspiration biopsy (FNAB).
In North America, FNAB of superficial organs is done by trained pathologists to assess the adequacy of the specimen on the spot, triage the lesion based on air-dried Diff-Quick stained smears, and provide the clinician with a preliminary diagnosis.
To obtain sufficient material by FNAB, the cutting effect of the advancing sharp oblique needle tip produced by the needle being moved back and forth within the lesion plays a major role compared to negative pressure created in the syringe. Also important is moving the needle back and forth in various directions within the lesion to minimize sampling error.
There are many diverse organ-specific approaches to disease, and pathologists must have sufficient knowledge and be given a more important role in FNAB practice.
