Background and purpose: Kennedy-Alter-Sung syndrome (KAS) is an adultonset form of motor neuron disease, which have X-linked recessive inheritance. Recently androgen receptor gene mutations were regarded causative factor for KAS. We evaluated the effects of muscle strengthening exercise on functional recovery in patients with KAS. Methods: Seven patients, all men, diagnosed by genetic study were enrolled in this study. Patients did home exercise in a prescribed and supervised protocol including muscle strengthening exercise and endurance training. Also we interviewed compliance of home exercise and evaluated ADL score. Muscle strength in lower limbs was measured by manual muscle test and isokinetic dynamometer measurements (Cybex II). Results: Significant increase in strength was observed soon after rehabilitation program started and keep strength through this program. Improvement of ADL was observed in less progressed patients who regularly exercised at home. Decrease in strength was observed in one of patients who did not keep protocol. Conclusions: It is suggested that rehabilitation program including muscle strengthening exercise in KAS should be prescribed for patients especially in their early stage
