岩手県盛岡保健所管内11市町村において,85歳を対象に,口腔内気体における揮発性硫黄化合物(VSC)濃度の実態および口腔内診査所見との関連を検討した.168名を対象に,舌苔の付着状態の視診(舌苔スコア)と口腔内気体のVSC濃度測定を行った.VSC濃度測定には,口臭測定器Oral Chroma<sup>TM</sup>を使用した.有歯顎者に対しては,歯と歯周組織の状態も診査した.その結果,H<sub>2</sub>S濃度が嗅覚閾値(112ppb)以上の者は11%,CH<sub>3</sub>SH濃度が嗅覚閾値(26ppb)以上の者は28%,H<sub>2</sub>S濃度またはCH<sub>3</sub>SH濃度が嗅覚閾値以上の者は33%であった.無歯顎者でH<sub>2</sub>S濃度が嗅覚閾値以上の者の割合は,舌苔スコア2,3の者では11%と,スコア0,1の者の0%よりも高い傾向を示した.有歯顎者でCH<sub>3</sub>SH濃度が嗅覚閾値以上の者の割合は,CPI重度歯数が1本以上の者では55%と,0本の者の27%よりも有意に高かった.また,無歯顎者ではH<sub>2</sub>S濃度と舌苔スコアのSpearman順位相関係数が0.48,有歯顎者ではCH<sub>3</sub>SH濃度とCPI重度歯数の係数が0.30と両者とも有意であった.そして,多重ロジスティック回帰分析により,無歯顎者におけるH<sub>2</sub>S濃度の嗅覚閾値に関係する要因として舌苔スコアが,有歯顎者におけるCH<sub>3</sub>SH濃度の嗅覚閾値に関係する要因としてCPI重度歯数が選択された.これらのことから,無歯顎者のH<sub>2</sub>S濃度には舌苔の付着状態が,有歯顎者のCH<sub>3</sub>SH濃度には歯周組織の状態が関連すると認められた.

The objective of this study was to measure the concentrations of volatile sulfur compounds (VSC) in mouth air from people aged 85 years, and to evaluate the relationship between VSC concentrations and oral health status. Tongue coating status (tongue coating score) and VSC concentrations in mouth air were assessed in 168 subjects The concentrations in each subject were measured with a VSC monitor (Oral Chroma<sup>TM</sup>). In addition, dental condition (decayed teeth) and periodontal condition (CPI Codes 3 and 4 recording teeth) were assessed for dentulous subjects. The results were as follows. 1. The percentage of subjects with an objectionable level of hydrogen sulfide (H<sub>2</sub>S≧112 ppb) was 11%, that of methyl mercaptan (CH<sub>3</sub>SH≧26 ppb) was 28%, and that of H<sub>2</sub>S or CH<sub>3</sub>SH (H<sub>2</sub>S≧112 ppb or CH<sub>3</sub> SH≧26 ppb) was 33%. 2. The percentage of edentulous subjects with an objectionable level of H<sub>2</sub>S was 11% in subjects with tongue coating score 2 or 3, while it was 0% in the subjects with tongue coating score 0 or 1. The percentage of dentulous subjects with an objectionable level of CH<sub>3</sub>SH was 55% in the subjects with deep pockets, while it was 27% in the subjects without. 3. There was a significant correlation between H<sub>2</sub>S concentration and tongue coating status in edentulous subjects (Spearman's ρ=0.48, p<0.001), and between CH<sub>3</sub>SH concentration and periodontal condition in dentulous subjects (Spearman's ρ=0.30, p<0.05). 4. In the multiple logistic regression analysis, the tongue coating status was significantly related to an objectionable level of H<sub>2</sub>S in edentulous subjects. On the other hand, the periodontal condition was significantly related to an objectionable level of CH<sub>3</sub>SH in dentulous subjects. The results of this study suggested that there was an association between H<sub>2</sub>S and tongue coating in edentulous elderly people, and between CH<sub>3</sub>SH and periodontal disease in dentulous elderly people.
