As antihypertensive drug is one of the most popular medications in general practice and used for very long term, it is essential to avoid its adverse reactions. Cautions in choosing the drugs, such as contraindication, unrecommended combination, organ complication, drug interaction and renal dysfunction, are discussed. Calcium-antagonists and angiotensin-converting enzyme inhibitors (ACE-I) are commonly used in Japan, thus we should have full knowledge of their side-effects. Some of the antihypertensive drugs are excreted via kidney. Most of ACE-I's and a part of beta-blockers, centrally acting agents are accumulated in blood when used for long time in patients with renal dysfunction. Thus, it is very important to adjust the dose of these drugs for patients with diseased kidney or with old age.
