Difficulties and problems exist in maintenance of oral hygiene in severely disabled patients (SDP). Investigations were made to assess the condition of oral hygiene and the prevalence of periodontal disease in SDP who were hospitalized in two infirmaries in Miyagi prefecture. The ages of 171 patients were ranged from 6 to 44 (average age was 21). The items investigated were the plaque index (Pl. I.), the calculus index (C. I.), probing pocket depth (P. D.), the salivary occult blood level, the oral hygiene level and the dietary conditions.<br>The aged patients had higher the C. I. than the younger patients. The group of patients who were administrated the DPH had higher P. D. than the non-DPH group. The differencies between the two infirmaries appeared in the P1. I., the C. I., the gingival bleeding index and the oral hygiene level. Most of the patients received a brief tooth-brushing from a nurse in charge.<br>High scores in SDP, indicating existence of advanced periodontitis, must be resulted from the inadquate oral hygiene procedures, such as, the less tooth-brushing time, difficulties in tooth-brushing, because of the patient's refusal, and less masticatory movements depending on having soft food.<br>Therefore, in SDP, an effective and simple procedures especially of oral hygiene must be established.
