Bronchial Embolization Prior to the Thoracoplasty for Pulmonary Aspergillosis

Abstract

Although cavernostomy with thoracoplasty is a therapeutic option for pulmonary aspergilloma, the bronchial fistulae often relapse after surgery. We report a case who underwent bronchial embolization with Endobronchial Watanabe Spigot (EWS) for the reduction of postoperative air-leakage. A 56-year-old male was admitted due to hemoptysis from pulmonary aspergilloma. The massive bleeding didn't cease after cavernostomy. We bronchoscopically placed 6 EWSs and air-leakage was apparently decreased, and then we conducted thoracoplasty and muscle prombage. Although the relapse of interstitial pneumonia led treatment failure, bronchial embolization had worked. The technique may be an option of preoperative care to decrease the postoperative air-leakage in thoracoplasty.
