Abstract

本研究は質的記述的研究であり,その目的は在宅移行期にある家族介護者が生活を立て直すプロセスについて明らかにすること,および家族介護者にとって安定した生活とは何かについて記述することである。家族介護者は初めて介護をする人で退院する病院と提携している訪問看護ステーションの利用者である。また,要介護者は退院後1年未満の65歳以上の高齢者で,家族介護者は同居家族とした。研究参加者は8名得られ,半構成的面接調査を実施した。分析の結果,家族介護者が生活を立て直すプロセスには4つのカテゴリーが見出された。それは【多重生活の調整と配分】【専門職との付き合い方の模索】【一人で格闘することから生まれる視野の拡大】【病人と向き合う】である。これら4つのカテゴリーは時間的経過を伴って変化し相互作用していた。その結果,家族介護者は【新しい自分の生活が回りだす】という感覚を得ていた。家族介護者はこのプロセスを通して,自分と他者との関わりを,時間や自分の気持ちを切り替えることによって,自分を取り戻し,自分のやり方を見出していた。これまで在宅移行期における家族介護者の生活を支援することは,生活の安定を取りもどすことであると理解されてきたが,それは専門家の見方であった。本研究の結果,彼らの生活認識は安定,不安定という状況ではなく,主体的に新しい生活の感覚を得ることであった。今後,家族介護者への適切な支援をするためには,家族介護者の生活の立て直しのプロセスを理解し,新しい生活で何をつくり出しているのかを明らかにする必要性が示唆された。

The purpose of this qualitative descriptive study was to clarify the life reorganization process of family caregivers during their family member's transition from hospital care to home care and to describe what is a well-balanced life for a family caregiver. First-time caregivers whose family member was receiving care from a visiting nurse station were recruited. The caregiver and care receiver were to be living in the same household ; care receivers were to be 65 year of age and older and their hospital discharge within the previous year. From semi-structured interviews that I conducted with eight caregivers I identified four processes within the larger process of life reorganization : "coordinating and managing time for their daily activities, " "finding ways to establish relationships with health professionals, " "devoting themselves to taking care of the care receiver, " and "being with and facing the care receiver sincerely." The gradual and evolving interaction of these processes led the caregivers to gain the sense that "one's new life is beginning" and a sense of their life "flowing well." Through this personal process the family caregivers recovered their sense of themselves ; by allocating time and mind between themselves and others and switching their attention among many considerations and activities they developed a sense of satisfaction. The life of family caregivers was thought by care professionals to be unstable during the transition from hospital care to home care. However, my conclusion is that, while their lives may not be stable, their instability is different from the idea of care professionals. Rather, the instability is due to the personal life reorganization process and finding a new sense of life for oneself. Further understanding the life reorganization process and its outcomes in new ways of life require further study so that health professionals might provide appropriate support to families.
