Incidence of caries has been on the decrease recently worldwide, and the number of caries requiring treatment has decreased in our country as well. Prevalence of caries in three years old children is rapidly decreasing and according to the report has decreased to 37% upon the odontopathy investigation of actual conditions in 1999. The significance placed on the removal of caries lesion and prosthodontics was overemphasized in regards to treatment in the last century. However, in the 21st century, it is important we also consider prevention and management of caries and remineralization of the enamel subsurface decalcified lesion. The enamel sub-surface lesion (white spot) is often seen in children and is susceptible to various changes in pathological condition, such as progress, arrest, and recovers. These changes cause difficulties in diagnosis and treatment. As fluoride and chemical treatments result in remineralization of the enamel subsurface lesion, careful diagnosis and observation of the lesion are important for successful clinical treatment. However, it is very difficult for the dentist to give an accurate diagnosis of the white spot. This is partly due to their diagnosis being largely dependant on their own subjectivity and preconceptions. A new method for detecting an enamel subsurface caries lesion has been developed in recent years using a laser. In this report, we introduce the new laser method (QLF method) and state the significance of managing the enamel subsurface lesion.
