生命保険と脳神経疾患 : 高齢化社会と技術の進歩  [in Japanese] Life Insurance and Neurological Disorders : An Aging Society and the Advancement of Technology  [in Japanese]

Abstract

日本人の高齢化,および各種診断・治療技術の進歩に伴い,保険金・給付金支払いの構造にも変化が生じている。高齢化に伴う入院・介護・災害死亡リスク,手術適応の拡大に伴う手術給付金など,生前給付が増加する一方,死亡リスクの先送りという側面もある。高齢化に伴う脳神経領域の代表疾患には,アルツハイマー型認知症やパーキンソン病などの変性疾患,脳血管性認知症や脳卒中などの血管病変があるが,診断技術の進歩により無症候性疾患(脳梗塞,未破裂動脈瘤,内頚動脈狭窄症,脳腫瘍など)も増加しており,引受査定時に頭を抱えることも少なくない。また,低侵襲治療による手術適応の拡大から,特に高齢者の血管病変や脳腫瘍の治療例が増加している。本稿では,主に高齢者と無症候性の脳神経疾患について,最近の知見を中心に総括する。後期高齢者医療制度の二の鉄を踏むことなく,高齢者にもやさしい保険商品の開発,査定の一助となれば幸いである。

Changes in the payment structure of the insurance and benefit claims have come out with Japanese's aging population and the advancement of technologies of various diagnoses and treatment. While the risk of hospitalization, nursing care and accidental death owing to the aging of the population rises, and the living benefits payment such as a surgical insurance benefit owing to the enlargement of therapeutic adaptation increases, there is the aspect of putting off the death risk. The major Neurological disorders with the aging of the population include degenerative diseases such as Alzheimer's disease and Parkinson's disease, and vascular lesion such as vascular dementia and stroke, but asymptomatic diseases (cerebral infarction, unruptured cerebral aneurysm, internal carotid artery stenosis, brain tumor etc.) also increase due to the advancement of diagnostic technologies, so that we are often in deep thought about these diseases when underwriting. The enlargement of the therapeutic adaptation by the minimally invasive surgery brings the increase of the cases that in particular elderly people underwent medical treatment for vascular lesion and brain tumor. In this paper, asymptomatic Neurological disorders in elderly people is mainly summarized focusing on the latest knowledge. I would be grateful if this study will contribute to the development of the elderly-friendly insurance products and the underwriting without making the same mistake as the health care system for the latter-stage elderly people.
