Abstract

本研究は混合歯列の子供に硬性ガムによる1か月間の咀嚼トレーニングを行い,咀嚼運動,最大咬合力および口唇閉鎖力がどのように変化するかを調べることを目的とした.被験者は本学付属病院矯正歯科に来院し,経過観察中の13名(女性9名,男性4名)の混合歯列期(Hellmanの歯齢III B~III C)の患者(平均年齢10.5歳)である.被験者には咀嚼トレーニングとして1日2回朝夕食後の10分間,硬性ガム(噛むブラッシングガム,(株)LION)を噛むことを指導した.咀嚼運動の評価を,咀嚼トレーニング開始前(T1),終了時(T2),終了後1か月経過時(T3)に,ナソヘキサグラフシステムにて主咀嚼側における軟性ガム咀嚼時の開閉口路角度,咀嚼経路幅,咀嚼サイクル数,ならびに左右咀嚼側の変化を測定し,さらに主咀嚼側の最大咬合力と口唇閉鎖力を測定した.その結果,T1とT2の比較から咀嚼経路幅,閉口路角度,咀嚼サイクル数,左右咀嚼側の変化と最大咬合力,ならびに口唇閉鎖力が有意に増加した.また,T1とT3の比較では咀嚼経路幅,左右咀嚼側の変化,最大咬合力,ならびに口唇閉鎖力が有意に増加した.一方,T2とT3の比較では咀嚼経路幅,左右咀嚼側の変化,最大咬合力,ならびに口唇閉鎖力に有意差は認められなかった.以上の結果から,硬性ガムを使用した咀嚼トレーニングによって咀嚼運動のグラインディングタイプ咀嚼への変化,咬合力と口唇閉鎖力の増加がみられ,終了後1か月においても維持されていた.

The purpose of this study was to investigate the changes of masticatory movement, maximum bite force, and labial-closure-strength after masticatory exercise using hard chewing gum for 1 month. The subjects were 13 children (9 girls, 4 boys; average age, 10.5y) with mixed dentition. The subjects practiced for 10 minutes twice/day for a period of 1 month. Chewing gum was used as the test product to conduct masticatory exercises. Jaw movements such as the opening and closing paths angle and masticatory width of the frontal plane on the habitual chewing side, number of masticatory cycles, and changes in right and left chewing sides were measured. Maximum bite force on the habitual chewing side and labial-closure-strength were also measured. Data were taken before starting exercises (T1), immediately after commensing exercises (T2), and 1 month after exercises (T3). There were significant differences between T1 and T2 in masticatory width, closing paths angle, number of masticatory cycles, changes in right and left chewing sides, maximum bite force, and labial-closure-strength. Furthermore, there were significant differences between T1 and T3 in masticatory width, changes in right and left chewing sides, maximum bite force, and labial-closure-strength. On the contrary, however, there was no significant difference in these parameters between T2 and T3. In conclusion, this study indicates that masticatory function is improved by masticatory exercises and that the beneficial effects were maintained ≤1 month after completion of masticatory exercises.
