母乳中免疫物質と母親のストレスとの関連に関する文献検討  [in Japanese] Review of the literature regarding the association between immune substances in breast milk and mothers' stress  [in Japanese]

Abstract

<目的>免疫産生能が未熟な新生児にとって,母体からの受動免疫が重要であることは周知のとおりである。母乳を介して児に移行すると考えられる免疫物質は,母親のストレス状態によってその濃度が増減するとの研究報告が散見される。本研究の目的は先行研究論文を試料とし,母乳中免疫物質と母親のストレスとの関連およびその研究方法について整理,検討することにある。<方法>1983〜2006年までの期間に,母乳中免疫物質と母親のストレスとの関係について研究された国内文献を「医学中央雑誌web版Ver.4」を用いて「母乳」「母親のストレス」「免疫」をキーワードとして検索,海外文献を「Pub med」を用いて「milk immune」「stress」「Stress of mothers during the puerperal period」「milk immunological substance」「the effect of postpartum stress」をキーワードとして検索,抽出した。<結果>母乳中免疫物質と母親のストレス要因について研究された論文は13件あった。ストレスとの関連が検討された因子は,母親の「栄養状態」をはじめ11因子であった。一方,母乳中免疫物質としては主に「SIgA」が指標として取り上げられており,評価時期は産後48〜72時間から産後1週以内が6件,産後2〜3ヵ月が6件,4〜6ヵ月が1件であった。<考察>母乳中免疫物質と母親のストレスとの関連についての研究は多いとはいえない。また,指標とする免疫物質の種類,母親のストレスの種類および調査時期と,おのおのの関連などについて幅広く検討する必要がある。

<Purpose> Passive immunity derived from breast milk is important for neonates, in whom the immune system is still immature. Several studies reported that concentrations of immune substances, which may transfer to neonates via breast milk, depended on the mothers' stress levels. In this study, we reviewed the association between immune substances in breast milk and mothers' stress, and the study methods based on the results of previous investigations. <Methods> We investigated and extracted literature published between 1983 and 2006 regarding the relationship between immune substances in breast milk and mothers' stress, via the "Central Medical Journal Web Ver. 4", using with "mother's milk" "mother stress" "immunity" as the keyword, and foreign literature via "PubMed", using "milk immune" "stress" "Stress of mothers during the puerperal period" "milk immunological substance" "the effect of postpartum stress" as key words. <Results> There were 13 articles investigating immune substances in breast milk and factors related to mothers' stress. Eleven factors including mothers' nutrition were examined. For immune substances in breast milk, SIgA was mainly employed as a parameter. In addition, the timing of evaluation was 48 to 72 hours to 1 week after delivery in 6 articles, 2 to 3 months after delivery in 6, and 4 to 6 months in 1. <Discussion> A few studies have examined the association between immune substances in breast milk and mothers' stress. In the future, the association between individual factors should be investigated by changing the type of immune substances used as a parameter, kind of mothers' stress, and timing of investigation.
