抄録

Initial success rate of magnetic compression enteroenteric anastomosis is high, but acute stenosis occurred unfavorably sometimes. If magnets migrate too early from the anastomosis, mucosal defects cause ulceration leading to cicatricial contraction and acute stenosis. To prevent this problem, we developed the Eigorotta Yamachicchi (EY) Stent The EY Stent consists of a pair of cylindrical stents, each with a rare earth disc magnet and a small flange. Upon completion of anastomosis, resistance occurs at both ends of the flanges and the mated stents are thought to retain longer at the anastomosis. We performed magnetic compression gastro-duodenal anastomosis by oral insertion of the EY Stent to six dogs. Three dogs were killed after two weeks and the other three dogs after four weeks. In the two-week group, we found that the Stents retained and ulcers at the anastomosis were identified. In the four-week group, the Stents were excreted before the dogs were killed and complete mucosal unions were confirmed. Leak-free anastomosis was formed in both groups. Compared with our other compression gastro-duodenal anastomosis dog experiments using ordinary magnets, the EY Stents retained longer and complete mucosal unions at the anastomosis were found. The result suggests that the newly developed EY Stents could be useful in preventing acute stenosis after enteroenteric anastomosis.
