Abstract

本研究の目的は,妊娠初期の抑うつ状態の妊婦の心理状態について検討することである。東北地方の2次救急医療を担っている病院の周産期医療センター外来において,妊娠15週未満で,本研究に同意の得られた初産婦に対し,自記式の外来患者用不安・抑うつテスト(hospital anxiety and depression scale)を依頼し,8〜10点を得点した「疑診」の初産婦5名を対象に不安や悩みについて半構成化面接を行い,内容分析の手法をもとにして分析を行ったところ以下の示唆が得られた。1)妊娠初期の抑うつ状態の妊婦は,【達成感がない】【負担感】【相談できない辛さ】【夫に対するアンビバレントな思い】【母になる悩み】【これでいいんだという思い】という要素のうち,複数を併せもっていた。2)妊娠初期の抑うつ状態の妊婦の心理状態を構成している各要素はそれぞれに関連して連鎖しているものがあった。抑うつ状態の妊婦達は適切なケアを受けることができない場合,病的な抑うつ状態に移行してしまう可能性があり,これら抑うつ状態の妊婦のスクリーニングと病的な抑うつ状態に移行することを予防するケアの必要性が示唆された。

The purpose of this research was to study the mental state of depressed pregnant women in the early stages of pregnancy. Permission to participate in this research was obtained from outpatients from the perinatal care centers of hospitals, at less than the 15th week of pregnancy, and they were requested to perform a self-administered, anxiety-depression test for outpatients (hospital anxiety and depression scale) and five primiparae who scored 8 to 10 points (marginal), were then given a semiorganized interview regarding anxiety and problems, and based on a method of analysis of the details, analysis was performed and the following implications were obtained. 1) Regarding depression in the early stages of pregnancy, amongst the factors, "no sense of accomplishment", "feelings of burden", "hard time not being able to discuss it", "feelings of ambivalence towards husband", "worries about becoming a mother" and "melancholy feeling that things are ok as they are", pregnant women were found to have multiple factors concurrently. 2) Of the factors that contribute to the mental state of depressed pregnant women in the early stages of pregnancy, there are those that are closely connected and related. If pregnant women who are depressed are unable to receive appropriate care, there is the possibility that it will progress to an abnormal depressed state, and thus it has been show that screening of these depressed pregnant women and prevention of the progression to an abnormal depressed state is required.
